- The user must click the “Apply Now” button on the main page to access the online membership application. Fill out the form with accurate information. Make sure the details are correct before submission.
- Applicants for membership must have a digital copy of the following documentary requirements prior to applying to the online application:
- 2” x 2” ID PictureProof of Payment (picture of the deposit slip or a screenshot of the online banking transaction )A copy of ONE of the following:
- Birth CertificateMarriage Contract
- ID
- TINSSS/GSIS
- 2” x 2” ID PictureProof of Payment (picture of the deposit slip or a screenshot of the online banking transaction )A copy of ONE of the following:
- IDPassport
